Please refer to the local campaign timeline page for deadline dates pertaining to toy applications before completing this form.

This form accepts Toy requests from families, individuals, and Nor-Profit Organization. Unless otherwise noted, the campaign accepts toy requests for children in the age groups provided.

In an effort to streamline our processes and help more children, we ask that you put "APPROVED LAST YEAR" in the note section if your family was approved last year. Also we will have a distribution with the kids present this year. Please adhere to instructions below:

Adults registering children must be Parents or Legal Guardians of the children.
Parents will be pick up/bring kids on their assigned Distribution Day.
Wheather children should come to pick up toys will be decided later.
Parents must have been APPROVED prior to coming to distribution.
Documentation must have been uploaded and verified in the time allowed.
*** APPROVED CONTROL NUMBER MUST BE PRESENTED TO RECEIVE TOYS ***
Distribution Time and Location will go out in an email.

In order to serve as many children as possible, we continue to ask that families only sign up for ONE toy program.

DO NOT SUBMIT MULTIPLE APPLICATIONS.
